Notice of Privacy Practices
Your health information is private and protected by the federal HIPAA law. This notice explains where to find how it is used and protected.

Your rights under HIPAA
- Ask for a copy of your health record.
- Ask to correct information you believe is wrong.
- Ask to limit how your information is used or shared, and get a list of who it was shared with.
- Get a paper copy of this notice.
- File a complaint if you believe your privacy rights were violated. You will not face retaliation for doing so.
